Privacy Notice Statement for Marketplace Qualified Health Plans (QHPs)
We are authorized to collect personally identifiable
information (PII) from you by the Centers for Medicare and Medicaid Services
(CMS). Any PII we collect is used to assist you in (a) applying for Marketplace
coverage or financial assistance; (b) enrolling in a Marketplace qualified
health plan; and (c) checking the status of your coverage and making updates
throughout the year.
If you choose to give us PII, we may share this information
with CMS and/or the insurance carrier you enroll with. CMS will maintain this
information in a federal System of Records. PII is used or disclosed only under
the following circumstances: apply for health insurance coverage or financial
assistance, determine eligibility for health coverage; determine eligibility
for premium tax credits and cost sharing reductions; choose an insurance plan;
enroll in a health plan; check the status of coverage; and make updates to your
health insurance application.
Any PII you provide is voluntary. If you choose not to
provide us with the PII requested, or not respond to certain questions, we may
not be able to help you enroll in a qualified health plan through the
